Your name as you would like it to appear in the book, if any of your contributions are selected for the book.
Bio
Tell the reader about yourself. Ideally in fewer than 100 words — and certainly no more than 200 — and with no bullets or additional subsections.

Your email address (either a mailto: tag or a bot-unfriendly description of your address) or web site via which you can be contacted. Your contact info will not be included in the book unless you mention it explicitly in the Biography section.
Image
A photo of you or an image that represents you. It should be a square with a minimum size of 300×300 pixels (i.e., 1.5"×1.5"/38mm×38mm at 200 dpi, 2"×2"/51mm×51mm at 150 dpi, 4.2"×4.2"/106mm×106mm at 72 dpi). Basically, you need to keep in mind that your image should look good if reduced to 1"×1"/25mm×25mm on a printed page — if your picture is dominated by a beautiful landscape, then what the reader will see is a stamp-sized version of landscape, but not you — it needs to include a decent head shot.
